To make your visit easier, come prepared by printing and completing our registration forms below.

If you have visited the clinic within the past year, you do not need to complete a new form. However, if you have a change or address, number, and/or name, please let us know at the time of your visit.
Please be prepared to show your student ID upon check in.
Are you enrolled into the HPU Health Clinic Service?
To double-check, please review your semester's statemetn or you may call us and we can look through our provided student list.
Not enrolled or don't want to enroll? No problem!
You can enroll with us in clinic, just let one of our team members know.
OR
We offer a fee for service that ranges between $75-$125 per visit (additional fees may apply for any lab or outside services needed for your visit).

Please complete this form which allows us to release information to only those listed on this form.
For Minors
If you under the age of 18, please send this document to your parents or guardians to complete. We require this before we can treat you at the clinic.
